Patient Financial Policy

This financial policy encompasses the following “provider entities”. Each of these providers will be submitting claims to your insurance company independently.

MEDICAL DEVELOPMENT CORPORATION
FACILITY FEES (OPERATING ROOM)
AMERICAN MEDICAL CARE, INC.
(ANESTHESIA FEES)

The practice of medicine is not an exact science and the medical treatment prescribed is done with the best of intentions and consistent with the highest standards applicable to the profession. There is no guarantee or promise as to the result that may be obtained. Payment of your medical bills is not contingent on outcome and payment is your responsibility once treatment is rendered regardless of such outcome.

The three companies listed above will submit your medical claims to your insurance company. None of the three companies listed above participate with any health insurance or managed care plans such as HMO, PPO or POS plans. Your insurance policy is a contract between you and your insurance company. None of the three companies listed above is a party to that contract. Insurance companies will not guarantee payment of claims. Not all services are covered in all contracts. Some insurance companies select certain services they will cover based on your individual plan coverage. Pre-operative services such as your chest x-ray, EKG and lab work may not be covered by your insurance policy. If your insurance company does not pay, then you are ultimately responsible for payment.

You will be billed separately for your lab work & pathology services. You must contact this provider directly regarding their charges.

If your insurance company denies coverage on a particular service, you will be responsible to pay for that service directly. Patients without insurance are required to pay the bill in full prior to the service being rendered. Any charges quoted to you are estimates. When treatment is completed actual charges will be generated.

You may request a personalized estimate of reasonably anticipated charges for the treatment of your specific condition. You may contact your health insurer or PPO for additional information concerning your share of costs (e.g. Deductibles, co-pays or out of pocket expenses). You will receive monthly statements from each entity listed above which will reflect your charges and payments.

The Agency for Health Care Administration places an emphasis on health care transparency for Florida’s consumers. The agency has create a website with detailed information on healthcare pricing and quality at locations and facilities around the state. This website is a step forward in allowing Floridians to make more informed health care decisions.

Pricing Transparency Requirements

The State of Florida requires most healthcare facilities to provide information about pricing and billing on their websites. Much of this information can be found on the Florida Health Pricing website of the Agency for Health Care Administration (AHCA). The site, which can be found at https://pricing.floridahealthfinder.gov shows average payments made to health plans for more than 200 service bundles. The bundles include payments to physicians, hospitals and ancillary services.

The service bundle information is a non‑personalized estimate of costs that may be incurred by the patient for anticipated services. Actual costs will be based on services actually provided.

Patients have a right to request a personalized estimate from the center. Patients and prospective patients should contact each health care practitioner who will provide services in the ASC to determine the health insurers and health maintenance organizations with which the health care practitioner participates as a network provider or preferred provider.

You may pay less for this procedure or service at another facility or in another health care setting.

You should contact your insurer or health maintenance organization regarding your cost‑sharing responsibilities.

Quality Measures

AHCA also provides information on quality measures on its web site at FloridaHealthFinder.gov. This website includes information on readmissions data, mortality rates, complication rates, infection rates, and patient satisfaction.

Financial Assistance Arrangements

We only schedule procedures at this facility by physicians who are on the medical staff at the facility. If your physician has determined that special financial assistance may be warranted and the physician agrees to those special financial arrangements for his or her services, you may be eligible for special financial assistance at the facility. If your physician or the physician’s office staff has agreed to provide special financial assistance to you for a procedure the physician wants to schedule at this facility, please contact us. We will require you complete an application for financial assistance that provides us information about your income and expenses. This will allow us to access your need and qualifications for special financial assistance. We follow the most recent poverty guidelines set by the U.S. Department of Health and Human Services.
